WEDDINGS AND PROMS: Weddings and proms have special packages. Failure to notify the sales representative about the event may result in adjustments to your reservation after the service, to reflect special rate. CANCELLATIONS: To avoid a late cancellation or no show fee, the reservation must be cancelled in accordance with the cancellation policy noted in your email confirmation. You must notify us of cancellation via email or phone. We are not liable for undelivered or blocked emails or faxes, please verify your cancellation over the phone if no cancellation confirmation is received by you. Full charge will be applied to your account in case of late cancellation or "no show". Unless stated otherwise in your email confirmation, cancellations made less than the following minimum time prior to the service date will be subject to full reservation price: for weekend limousine service and coach buses 1 week, for party buses 2 weeks, for holidays, special occasions and events 1 month for all services. For airport transfers cancellations and changes must be completed at least 24 hours in advance. If you experience difficulty locating the chauffeur, please call the Customer Service telephone number listed on the email confirmation. Leaving the pick-up location without notifying Memphis LX Limo will result in a no-show charge. Cancellation policy comes in effect after submition of online reservation. By making reservation you agree to complete "Terms and Agreements" that you can see at http://FortWorthlxlimo.com/Home/tabid/894/ctl/Terms/Default.aspx ADDITIONAL CHARGES: Tolls, parking, phone usage, wait time, gratuity, TTC (as defined below), airport fees and other surcharges may not be included in the estimate may also apply to the final price. DEPOSIT: No deposit required for airport services and hourly services under 6 hours. All services over 6 hours will require $300 deposit. All prom/homecoming and special services will require $500 deposit. Deposits apply towards outstanding balance on the reservation and are non refundable in case of cancellation.

Data Springs Inc. DYNAMIC FORMS LICENSE AGREEMENT PROVISIONS Software License Agreement 1. This is an agreement between Licensor (Data Springs Inc.) and Licensee, who is being licensed to use the named Software (“Dynamic Forms”). 2. Licensee acknowledges that this is only a limited nonexclusive license. Licensor is and remains the owner of all titles, rights, and interests in the Software. Licensee may not sell, distribute, or package for sale named Software. 3. This License permits Licensee to install the Software on one installation of DotNetNuke® without additional fees or licenses. Licensee will not make copies of the Software or allow copies of the Software to be made by others, unless authorized by this License Agreement. Licensee may make copies of the Software for backup purposes only Note: If module software source code was purchased the changes made to the source code/recompiled version are still licensed only to one installation of DotNetNuke®. 4. This Software is subject to a limited warranty. Licensor warrants to Licensee that the physical medium on which this Software is distributed is free from defects in materials and workmanship under normal use, the Software will perform according to its printed documentation, and to the best of Licensor’s knowledge Licensee’s use of this Software according to the printed documentation is not an infringement of any third party’s intellectual property rights. This limited warranty lasts for a period of 30 days after delivery. To the extent permitted by law, THE ABOVE-STATED LIMITED WARRANTY REPLACES ALL OTHER WARRANTIES, EXPRESS OR IMPLIED, AND LICENSOR DISCLAIMS ALL IMPLIED WARRANTIES INCLUDING ANY IMPLIED WARRANTY OF TITLE, MERCHANTABILITY, NONINFRINGEMENT, OR OF FITNESS FOR A PARTICULAR PURPOSE. No agent of Licensor is authorized to make any other warranties or to modify this limited warranty. Any action for breach of this limited warranty must be commenced within one year of the expiration of the warranty. Because some jurisdictions do not allow any limit on the length of an implied warranty, the above limitation may not apply to this Licensee. If the law does not allow disclaimer of implied warranties, then any implied warranty is limited to 30 days after delivery of the Software to Licensee. Licensee has specific legal rights pursuant to this warranty and, depending on Licensee’s jurisdiction, may have additional rights.
